Cigarette Stamp Credit Request

The distributor must submit a notarized affidavit, on company letterhead, attesting to the ownership of the product before it was sent back to the manufacturer.

The distributor must also submit a notarized credit memo and a manufacturer affidavit.

The information in the affidavit must match and confirm the credit memo.

Supported file formats:

.jpeg, .jpg, .tff, .gif, .png

Supported file size:

File size must be less than 5 MB per file. Multiple files are allowed

File:

Navigate to the file, select it, and choose Open to attach the file.

When you have uploaded your files, select Continue.
