Tax Assessor-Collector: An organization that has an officer (or employees) that enters into a contract to collect taxes on behalf of a taxing unit. Due to overlapping jurisdictions, a special purpose district may have more than one Tax Assessor-Collector.

Otherwise, provide the following:
Organization Name: The name of the company that has entered into an agreement with the special purpose district to execute tax assessor-collector duties.
First Name, Middle Name, Last Name: The name of the person who represents the main office of the organization.
Phone Number: The phone number of the organization’s main office.
Address, City, State and Zip Code: The mailing address of the organization’s main office.
Select Continue.
Utility Operator: A business or individual responsible for the day-to-day operations of a special purpose district, including billing, customer service, repairs and maintenance.
If a special purpose district does not contract with a Utility Operator, check the box and select Continue.
Otherwise, provide the following:
Organization Name: The name of the company that has entered into an agreement with the special purpose district to execute Utility Operator duties.
First Name, Middle Name, Last Name: The name of the person who represents the main office of the organization.
Phone Number: The phone number of the organization’s main office.
Address, City, State and Zip Code: The mailing address of the organization’s main office.
Select Continue.
