If the answer is yes, select the fireworks business type.

First Business Date: Enter the date of the first day of business.

Do you sell Fireworks on consignment? Select Yes or No.

If yes, enter the name and the taxpayer number for whom you are selling:

Distributor Name: Enter the distributor name.

Distributor Taxpayer Number: Enter the distributor taxpayer Number.
