Business Name: Business name for the business added.

Address: Address for the business added.

City: City for the business address added.

State: State for the business address.

Zip Code: Zip Code for the business address added.

Effective Date: Effective date of the business address added.

Edit: Select on Edit button to edit business information.

Delete: Select on Delete button to delete business information.

Add Another Business Location: Select on the Add Another Business Location button to create a location.
